Rates and Fees

The Personal Services Assessment
is done by the Administrator and Director of Nursing from The Villa At The Lake.
It determines the eligibility of a prospective resident and the Level of Care at which
they will be charged.
The Personal Services Assessment takes into consideration the following areas:
Mobility
Cognition/Psychosocial Orientation
Bowel/Bladder Control
Medication Administration
Special Supportive Services
Additional Activities of Daily Living (ADL's) including: Dressing & Personal Hygiene

A resident's rate is made up of the Level of Care required and the size of the room requested. Below is the rate chart for one resident in each size room at Levels 1 through 5.

Residency Fee
A non-refundable residency fee of $600.00 is due upon reservation of a permanent suite.

Furnished Rooms
Furnished rooms reserved for a respite or temporary stay are charged a furniture rental fee of $15.00 per day. If the resident decides to stay permanently this rental charge is waived.

Terms of Payment
First month's rental is due upon admission. Thereafter, a monthly statement is delivered or mailed by the 1st of the month due by the 10th of the same month.
A 3% service charge will be added for bills not paid by the 10th.
The Villa At The Lake is not a participant in the Medical Assistant Program administered by the Ohio Department of Human Services. A resident may be discharged from if they become unable to pay for the services provided.

Other Miscellaneous Charges
Chauffeur Service
: A nominal fee will be charged for service
beyond a 10 mile radius of The Villa.
Guest Meals*:
Breakfast $4.00
Lunch $8.00
Dinner $6.00
Special Events $12.00
Children 12 and under half price. *Meals
to be paid by the guests or charged to the resident's monthly bill.
T.B. Mantoux Test: $5.00/test
Blood Sugar: $5.00
Intermittent Injections: $6.50
Sundry Shop: Health and Personal Supplies available at competitive unit pricing.
Residents are able to pay for these with cash or charge them to their monthly statement.

Current residents will be given a 30 day notice.
